What types of health insurance do you accept?

I accept private pay and will provide you with a detailed bill to submit to your insurance company.

Do I need a doctor’s referral?
Yes, I required a doctor's prescription for physical and/or aquatic therapy.
What is your new patient procedure?
All prospective patients should email me regarding my therapy services. We can work out a day/time for an initial evaluation. Before the first day, please fill out new patient data sheet under the forms tab on the site menu. Once completed, this form can be faxed back to me at 703-935-8710. If you do not have the ability to fax, simply let me know that through email and I will call you to get the information. Prior insurance approval is required to initiate services.
